INSURANCE/BILLING

(I). Insurance Overview:

      BGPT accepts most major insurance carriers, including: Blue Cross Blue Shield, Blue Choice, United Healthcare, Humana, Aetna, Medicare, and Workers Compensation plans.  Our front desk staff will be happy to verify your health care benefits as it pertains to your physical therapy treatment coverage.

(II). Billing/Payment Policy:

     BGPT will file claims directly with your insurance carrier. Co-payments and co-insurances are due at the time services are rendered.  Deductibles will be collected from the patient upon receipt of the EOB (Explanation of Benefits) from the insurance carrier.  Processing time for the insurance carriers typically ranges from 3-8 weeks from the time of submission to the insurance carrier.  BGPT offers individualized payment plans, and will work closely with each patient to accommodate individual needs.  We accept cash, personal checks, Visa, MasterCard, and/or Discover.

Your First Visit - What to expect

Acceptable forms of photo identification include:

  • Valid State Identification Card
  • Valid State Driver’s License
  • Valid Passport
  • We ask you arrive at least 15 minutes prior to your scheduled appointment time in order to allow sufficient time to complete all necessary paperwork.
  • Please remember to bring your insurance carrier card and physician's referral for physical therapy.
  • Please wear comfortable clothing.
  • Please allow at least hour or more for your initial evaluation.
